Please join Program Manager Paula Wolfson for a review of Care Partners services and discussion on navigating life transitions. As a group we will try to pinpoint the types of services you find most helpful or what services you feel are needed. Paula hopes to learn from members about their challenges and how they define or experience a “transition.”
